Utah cards require annual renewal. We make it simple — and members pay only half the cost plus the required $8 state fee.

HOW IT WORKS:
1. Schedule your evaluation
Call or text us to book. Let us know whether this is a new evaluation or an annual renewal. We'll confirm your appointment, send you an electronic intake form and let you know what to bring.
2. Meet with Dr. Law
Dr. Law conducts a thorough evaluation to determine whether you qualify under Utah's qualifying conditions. This is a real clinical visit — he'll review your medical history and discuss how medical cannabis may fit your treatment.
3. We submit your recommendation electronically
If you qualify, we handle the state submission electronically on your behalf, and walk you through setting up the online accounts that the state requires you to create. No extra paperwork for you to navigate. We help you every step of the way. In general yo will leave your appointment in under 30 minutes with a temporary card in your inbox.
4. Your card arrives by mail
The Utah Department of Health & Human Services processes your application and mails your permanent Medical Cannabis Card. Processing times vary.
UTAH QUALIFYING CONDITIONS
Utah law requires a qualifying medical condition for a Medical Cannabis Card. Common qualifying conditions include:
Epilepsy / seizures
Multiple sclerosis
Crohn's disease
Chronic pain
PTSD
Cancer
Terminal illness
Rare conditions (case-by-case)
Autism
HIV / AIDS
Alzheimer's disease

* The $8 state fee is required by Utah law for all Medical Cannabis Card applications and renewals. It is not a practice fee and cannot be waived.
