SPORTS PHYSICAL
Our sports physical meets all Utah school and league requirements for student athletes in grades Kβ12 and beyond. Dr. Law conducts a thorough cardiovascular and musculoskeletal examination β not a quick checklist β and completes your official clearance form same day so your athlete can get back to what they love without delay. No follow-up visit required. Same day appointments welcome when availability allows, though we recommend calling or texting ahead to confirm. Missionary Physical FAQs

Missionary physicals are more comprehensive than sports physicals and typically take 45β60 minutes. The appointment includes a full physical examination, vision screening, hearing screening, EKG, urinalysis, immunization history review, and completion of all official church paperwork. Plan for a thorough visit β Dr. Law takes time to make sure everything is done right the first time.

Dr. Law completes all official Church of Jesus Christ of Latter-day Saints missionary health forms at your appointment. You will need to bring your church-issued health forms or access them through your church account before your visit. If you are unsure which forms are required, contact your bishop or stake president before scheduling β requirements can vary slightly depending on your mission call timeline.

Missionary physicals typically require a TB skin test, hemoglobin/hematocrit blood work, and a chest X-ray β these are completed at an outside facility and billed separately. We provide all necessary referrals at your appointment and can recommend convenient local facilities. Plan for these to be completed within the timeframe required by the church β we'll walk you through the timeline at your visit.

We recommend scheduling your missionary physical as soon as you receive your mission call β ideally 4β6 weeks before your report date. This allows time to complete the required outside tests (TB test, blood work, chest X-ray) and submit everything to the church before your departure. Same-week appointments are usually available but earlier is always better to avoid any last-minute stress.

Fasting is not required for the missionary physical itself. However if blood work is being ordered at the same visit that requires fasting, we will let you know when you schedule. Call or text us ahead of your appointment if you have questions about preparation.

Absolutely β parents and family members are welcome to attend if the missionary is comfortable with that. Many families treat the missionary physical as a meaningful milestone and we are happy to accommodate however many people you'd like to bring. Just let us know when you schedule so we have enough space.
Sports Physical FAQs

Same-week scheduling is usually available and same-day appointments are often possible depending on availability. Call or text 801-571-0796 to check current availability. We recommend booking at least a few days ahead during busy seasons β particularly late summer when school sports physicals peak.

Bring your completed sports physical form from your school or league if one is required β we'll fill out our section and sign it at your appointment. Also bring a list of any current medications and a brief health history if your athlete has any known conditions. If you don't have a specific form, we use a standard Utah sports clearance form that is accepted by most schools and leagues. Call or text us if you're unsure what your school requires.

Most sports physicals take 20β30 minutes from check-in to checkout. Dr. Law conducts a thorough examination β not a rushed checklist β so plan for a comfortable visit rather than a quick in-and-out. Your clearance form will be completed and ready before you leave.

Progressive Primary Care is a Direct Primary Care practice and does not bill insurance for any services. Sports physicals are $75 for non-members β a flat fee with no surprise bills. Members pay nothing. HSA and FSA funds may be used for this visit. Many families find our pricing is comparable to or lower than a typical insurance copay.

Yes β we can often schedule siblings back to back on the same day. Call or text us at 801-571-0796 to discuss availability. Families with multiple athletes should mention this when booking so we can reserve enough time.

A sports physical is focused specifically on evaluating whether your athlete is safe to participate in organized sports β it assesses cardiovascular health, musculoskeletal fitness, and any conditions that might affect safe participation. A regular checkup is broader and covers overall health, preventive screenings, vaccinations, and chronic condition management. If your child is due for both, ask us about combining them into one visit if your child is over 16. We donβt do pediatric visits.

Yes β in addition to sports and missionary physicals, Dr. Law conducts comprehensive annual physicals, executive physicals, and pre-employment physicals for adults. These are included in membership or available as a drop-in visit. Contact us to discuss your specific needs.

Our sports physicals are completed by an internal medicine physician and meet Utah state requirements for school and league athletic clearance. If your specific school, league, or organization has unique requirements, bring their form to your appointment and Dr. Law will complete it. If you are unsure, call us and we can confirm before you schedule.

BRINGING IN A FAMILY FOR A PHYSICAL?
Both physicals are included in membership β along with everything else in primary care.
If your family is already considering a primary care home, a missionary or sports physical is a great way to meet Dr. Law before committing. Members pay nothing for physicals β and also get unlimited visits, same- or next-day access, telehealth, and more for $49-99/month. Families with multiple athletes or a soon-to-be missionary will often recoup the membership cost in their first few visits.
