Preparing for a mission is one of the most meaningful milestones in a young person's life. The medical evaluation process doesn't have to be stressful. At Progressive Primary Care, Dr. Law conducts comprehensive pre-mission physicals in Sandy, Utah β completing all official Church of Jesus Christ of Latter-day Saints health paperwork at your appointment so your missionary can submit their health forms and move forward without delay.

Appointment time: Plan for 45β60 minutes. Dr. Law takes time to be thorough β this is not a rushed checklist appointment.
*Required test referrals: Missionary physicals typically require a TB skin test, hemoglobin/hematocrit blood work, and a chest X-ray. We provide all necessary referrals at your appointment. These tests are completed at an outside facility and billed separately.

Step-by-step: completing your missionary medical clearance
Here is exactly what to expect from start to finish:
Step 1 β Schedule your appointment at Progressive Primary Care
Call or text (801) 571-0796 or contact us online. Let us know you're scheduling a missionary physical so we can reserve enough time. Same-week appointments are usually available, sometimes we can even get you in same-day β we recommend scheduling as soon as your missionary receives their call.
Step 2 β Access your church health forms before your visit
Log into your church account at lcr.churchofjesuschrist.org and access the missionary health forms through the missionary portal. Bring the forms to your appointment so Dr. Law can complete the physician section at your visit.
Step 3 β Your appointment at Progressive Primary Care
Dr. Law conducts your full physical examination, completes all required church paperwork, and provides referrals for the outside tests required by the church β TB skin test, hemoglobin/hematocrit blood work, and chest X-ray. These outside tests are completed at a separate facility and billed independently.
Step 4 β Complete your TB skin test
The TB skin test is a two-visit process β the test is placed on day one and read 48β72 hours later. Schedule this immediately after your PPC appointment so it doesn't delay your submission timeline. TB skin tests are available at most urgent care clinics, health departments, and some pharmacies across the Wasatch Front.

Step 5 β Complete your blood work
Hemoglobin and hematocrit blood work is required. We provide the referral at your appointment. Many ARUP, LabCorp, and Quest Diagnostics locations across the Wasatch Front accept walk-in lab orders.

Step 6 β Complete your chest X-ray
A chest X-ray is typically required for missionary health clearance, but not always so confirm that itβs required for you before you get one. We provide the referral at your appointment. Many urgent care and imaging centers across Salt Lake and Utah Counties offer same-day chest X-rays without an appointment.
Step 7 β Schedule your dental assessment
The Church of Jesus Christ of Latter-day Saints requires a dental examination as part of missionary health clearance. This must be completed by a licensed dentist β we cannot perform the dental assessment at Progressive Primary Care, but we recommend scheduling it concurrently with your medical appointments to avoid any delays in submission.

Step 8 β Submit your completed health forms
Once all medical and dental assessments are complete and results are available, submit your health forms through your church missionary portal. Your bishop or mission president's assistant can confirm the submission process and deadline for your specific call.
Step 9 β Follow up if needed
If the church requires additional information or follow-up tests, contact us immediately β we'll turn around any needed documentation as quickly as possible to avoid delays to your mission call timeline.
How far in advance should I schedule?
We recommend scheduling your missionary physical as soon as you receive your mission call β ideally 4β6 weeks before your report date. This allows enough time to:

Many families treat the missionary physical as a meaningful milestone and we are happy to accommodate however many people you'd like to bring.
